agingCongregate Meals

Congregate meals are available at 25 different nutrition sites in seven Eastern Oklahoma counties. Registered Dietitians visit each site monthly to provide nutrition education materials and discussions. Click here for a list of Nutrition Centers.

Home Delivered Meals

Home delivered meals are delivered from 25 nutrition centers. These meals are for those who are considered homebound (unable to leave their home without the assistance of another person) and have no one in the home during the day to cook a meal for them. Project H.E.A.R.T. and SAC Nutrition make great efforts to serve all those in need of a home delivered meal. Unfortunately, there are not enough volunteers to deliver these hot noon meals. If you are interested in volunteering to deliver meals, please contact:

Outreach Services are available in each of the 7 counties. Outreach workers seek out those persons age 60 years of age and older and try linking them with services to assist them with living independently. To contact an Outreach Worker, contact SAC Outreach (918) 775-7708.

Health Promotion services are provided through Oklahoma Healthy Aging Initiative. Please contact (918) 779-7367 for more details.

Title III Case Management services are provided through EODD AAA. One part-time Case Manager provides assistance in the form of access or care coordination. Some Case Management activities include:

  • Assessing needs
  • Developing care plans
  • Authorizing services
  • Arranging services
  • Coordinating the provision of services among providers
  • Follow-up as required
